Treatment Overview
About Frenuloplasty
If you experience pain, tightness, or tearing on the underside of your penis during erections or sexual activity, you may have a short or tight frenulum. This is the small, elastic band of skin that connects your foreskin to the head of your penis, and when it is too short, the condition is known as a frenulum breve.
A frenuloplasty is a straightforward, minor surgical procedure designed to release this tension.Unlike a full circumcision, a frenuloplasty completely preserves your foreskin. Your surgeon will make a small incision across the tight band of tissue and stitch it lengthways, effectively lengthening the frenulum to allow natural, pain-free movement and flexibility.
Safety Information
Benefits, Risks & Side Effects
01
Benefits
Undergoing a frenuloplasty offers significant physical and emotional benefits, transforming your comfort and sexual health:
Pain-Free Erections: By removing the tension on the underside of the penis, erections become comfortable and expand naturally without pulling.
No More Tearing or Bleeding: Lengthening the tissue stops the repetitive, painful tearing that can happen during intimacy.
Better Foreskin Mobility: Your foreskin will slide back smoothly and comfortably, making everyday personal hygiene much easier.
Preserves Natural Anatomy: This is an ideal option if you wish to keep your foreskin intact and avoid a full circumcision.
Restored Confidence: Eliminating the anxiety of pain or bleeding during sexual activity brings peace of mind and improves your intimate relationships.
02
Risks
While a frenuloplasty is a routine and safe procedure, all surgical treatments carry a few risks.Fully aligned with the British Association of Urological Surgeons (BAUS) guidelines, potential risks include:
Bleeding: Minor bleeding from the cut can happen, which between 1 in 10 and 1 in 50 men experience.Rarely, this may require a quick follow-up procedure to stop.
Infection: A small risk of infection at the wound site, which can typically be treated easily with a course of antibiotics.
Altered Sensation: A rare risk (between 1 in 10 and 1 in 50) of permanently reduced or altered sensation in the glans (the head of your penis).
Scar Tenderness: The small scar left behind may feel slightly tender or firm initially, but this usually softens over a few months.
Unsatisfactory Cosmetic Outcome: There is a small chance you may not be fully satisfied with the final look of the area.
Need for Circumcision: If the procedure does not fully resolve your tightness or symptoms, a full circumcision may be required in the future.
03
Side Effects
Following your surgery, you can expect some temporary side effects as your body heals. These are entirely normal and will resolve on their own:
Swelling and Bruising: Your penis will look bruised and swollen. This is very common and typically lasts between 7 to 14 days.
Mild Discomfort: You may feel some soreness or stinging, especially when the anaesthetic wears off, which can be managed with simple over-the-counter painkillers.
Hypersensitivity: The head of your penis may feel highly sensitive or tender for the first two to three weeks as it adjusts.
Dissolvable Stitches: Your stitches will feel like small knots and will dissolve naturally, typically within two to three weeks, though they can sometimes take up to six weeks to completely disappear.

Patient Journey
What to Expect
A step-by-step guide through your Frenuloplasty journey—from preparation and what happens during the procedure to recovery and aftercare.
01/ 03
Before Procedure
To ensure your procedure goes smoothly, please follow these simple steps before your surgery:
Keep the Area Clean: Thoroughly wash your genital area on the morning of your procedure to minimise any risk of infection.
Review Your Medications: Inform your urology team about all medications you take. You may be asked to temporarily stop blood-thinning medicines (such as aspirin or warfarin) a few days prior.
Fasting: As the procedure is sometimes done under a general anaesthetic (where you are asleep), you will be asked not to eat or drink for 6 hours before your appointment.
Arrange a Ride Home: Because you will have a local or general anaesthetic, you must arrange for a partner, friend, or family member to drive you home after the procedure.
Wear Loose Clothing: Wear loose-fitting trousers and supportive, comfortable underwear to the clinic to avoid unnecessary pressure on the area afterward.
02/ 03
During Procedure
A frenuloplasty is a quick outpatient procedure that typically takes between 10 to 15 minutes:
Numbing the Area: The surgical site is cleaned, and you will receive a local anaesthetic injection to completely numb the penis, ensuring you feel no pain.In some cases, a general anaesthetic may be used so you are asleep.
Releasing the Tension: Once the area is fully numb, the surgeon makes a precise horizontal incision across the tight band of the frenulum to release the pulling sensation.
Lengthening the Tissue: The incision is carefully pulled into a diamond shape and stitched lengthways (vertically) using fine, dissolvable stitches. This technique adds length and flexibility to the skin.
Protection: A light, protective dressing or ointment may be applied over the stitches to protect the wound.
03/ 03
After Procedure
Your recovery at home is vital for achieving the best results. Please follow this expert aftercare advice:
Manage Discomfort: Use simple painkillers like paracetamol to manage any mild soreness.
Keep it Clean and Dry: Keep the area dry for the first 24 to 48 hours.Any initial surgical dressing should fall off within 24 hours; if it does not, or if it becomes wet with urine, gently remove it yourself.Avoid soaking in a bath or swimming for one to two weeks.
Use Petroleum Jelly: Apply a little Vaseline around the stitch line daily. This stops the wound from sticking to your clothing and reduces friction.
Gently Move the Foreskin: Once the initial swelling begins to calm down, gently pull back your foreskin on a daily basis to keep the tissue flexible and maintain the long-term benefits of the surgery.Only pull it back as far as feels comfortable.
Rest and Recover: Avoid heavy lifting or vigorous exercise for two to three weeks. You must refrain from all sexual activity (including intercourse and masturbation) for four to six weeks to allow the delicate tissue to heal fully.

A frenuloplasty will alter the appearance slightly by lengthening the small band of skin underneath, making it look relaxed rather than pulled tight. Unlike a circumcision, your foreskin remains fully intact. While the head of the penis may feel hypersensitive for the first few weeks, your long-term natural sensitivity and sexual sensation will be preserved.
