Treatment Overview
About Medical Treatment for Urinary Symptoms
Medical treatment for urinary symptoms involves using tablets to either relax or shrink the prostate, or to calm an overactive bladder. For many men, this is the first and most effective step in managing "Lower Urinary Tract Symptoms" (LUTS).
We use three main types of medication:
Alpha-Blockers (e.g., Tamsulosin): These work quickly by relaxing the muscles in your prostate and the exit of your bladder, making it much easier to pee.
Prostate-Shrinkers (5-ARIs like Finasteride): If your prostate is particularly large, these tablets work over several months to physically shrink the gland.
Bladder-Calming Tablets (Beta-3 Agonists or Anticholinergics): These help if you have a frequent, urgent need to go to the toilet by stopping the bladder from "squeezing" too early.
Safety Information
Benefits, Risks & Side Effects
01
Benefits
The primary benefit is getting your quality of life back without needing surgery.
Better Sleep: Reducing the number of times you have to get up in the night.
Confidence: Removing that sudden, "must-go-now" feeling that can interfere with your daily plans.
Safety: Starting treatment early can often prevent your bladder from blocking completely (acute retention), which would otherwise require an emergency trip to the hospital.
02
Risks
These medications are generally very safe and are used by millions of men across the UK. However, there are some specific points to be aware of. Alpha-blockers can occasionally cause a sudden drop in blood pressure, which might make you feel a bit dizzy, especially when you first stand up.
If you are taking prostate-shrinking tablets (5-ARIs), your specialist will monitor your PSA blood tests closely. It is important to know that these tablets will naturally "halve" your PSA reading, so your doctor must double your result when checking for prostate health. For bladder-calming drugs, we now prefer newer "Beta-3 Agonists" for older patients, as some older types of medication have been linked to memory or "cognitive" changes in later life.
03
Side Effects
Most side effects are mild and go away if you stop taking the medication.
Alpha-blockers: You may notice "dry orgasm" (where no fluid comes out during climax) or a slightly stuffy nose.
Prostate-shrinkers: About 3–5% of men notice a lower sex drive or difficulty getting an erection.
Bladder tablets: The most common effects are a dry mouth or occasional constipation.

Patient Journey
What to Expect
A step-by-step guide through your Medical Treatment for Urinary Symptoms journey—from preparation and what happens during the procedure to recovery and aftercare.
01/ 03
Before Procedure
Before you start, your GP or specialist will perform a few routine checks:
Health Checks: A simple blood test (PSA) and a quick physical check of the prostate (DRE).
Bladder Diary: You may be asked to keep a record of what you drink and how often you pee for a few days.
Flow Test: A simple test where you pee into a special machine to measure the strength of your stream.
Ultrasound: A quick, painless scan to see if your bladder is emptying fully.
02/ 03
During Procedure
This is a self-managed treatment that you take at home.
Routine: Most men take their alpha-blocker once a day, usually before bed, to help prevent any daytime dizziness.
Consistency: Prostate-shrinking tablets are also taken once daily.
Patience: While flow-improving tablets work in just a few days, shrinking tablets can take 3 to 6 months to show their full effect. It is vital to keep taking them daily even if you don't notice a change immediately.
03/ 03
After Procedure
We usually expect improvement of symptoms within days or weeks. Prostate shrinking tablets may take several months to lead to a response.
Ongoing Support: If your symptoms are well-controlled, you will typically only need a quick review with your GP once a year.

The timeline for symptom relief depends entirely on the specific type of medication prescribed. Alpha-blockers (such as Tamsulosin) work quickly by relaxing the muscles around the prostate and bladder neck; you will usually notice an improved flow within just a few days. Conversely, prostate-shrinker tablets (5-ARIs like Finasteride) work gradually over a longer period to physically reduce the size of an enlarged prostate. It can take between 3 to 6 months of daily use to see the full therapeutic effect of these tablets, so consistency is key even if you don't notice an immediate change.

Prostate-shrinking medications (5-ARIs) will naturally alter your PSA (Prostate-Specific Antigen) levels, typically halving your baseline reading after a few months of use. This is a normal and expected effect of the medication shrinking the prostate tissue. However, it is vital that any healthcare professional managing your care is aware you are taking this tablet. When monitoring your prostate health, your doctor or consultant urologist will need to mathematically double your PSA result to ensure it is interpreted accurately and that any potential concerns are not missed.

The vast majority of patients tolerate these medications very well, and any side effects are generally mild and fully reversible if the treatment is stopped.
Alpha-blockers can occasionally cause minor daytime dizziness (which is why we often recommend taking them before bed), a slightly stuffy nose, or "dry orgasm" (retrograde ejaculation, where little to no fluid is expelled during climax).
Prostate-shrinkers carry a small (3–5%) risk of causing a lowered sex drive or mild erectile difficulties.
Bladder-calming tablets may cause a dry mouth or occasional constipation. If you experience any bothersome side effects, your consultant can easily adjust your dosage or explore alternative treatment pathways.
